What we need to quote you
- 01Census. For each eligible employee: date of birth, home ZIP, gender, and whether they'd cover a spouse or children.
- 02Current plan, if any. Summary of benefits and current rates — this is what we have to beat.
- 03Your contribution intent. What percentage of the employee‑only premium you plan to cover. Most carriers require at least 50%.
- 04Target effective date. Group plans usually start on the first of a month; allow 3–4 weeks.
No employee health information is needed to start. If a level‑funded quote turns out to be worth pursuing, underwriting happens later — with the carrier, not with us.

This policy covers information we collect through cammaranohealth.com, our forms, phone calls, text messages, email, and online meetings. It applies to everyone who contacts us, wherever you live. We are licensed in 26 states, listed on our Licenses page.
Who controls your information. We collect and control the information you give us. We are not acting for any one insurance company when you fill out our forms. When you apply for a plan, the insurance company or Marketplace also receives your information. Each one gives you its own privacy notice. That notice governs how it handles your information.
Information we collect
In short, we collect these kinds of information:
- Contact details: name, email address, mobile phone number, ZIP code, and state
- Household and eligibility details: ages or dates of birth, who needs coverage, tobacco use, and life changes
- Business details: number of employees, industry, current coverage, and an employee census
- Health details: doctors, hospitals, prescriptions, and planned care
- Budget and money details: monthly budget, and household income if you share it for subsidy estimates
- Your consent record: what you agreed to about calls and texts, and when
- Technical details: IP address, browser details, and the page you came from
On our quote form, you tell us
- Who needs coverage: you, your family, or your business
- ZIP code, state, ages, number of people to cover, and tobacco use in the household
- For a business: number of eligible employees, industry, and current coverage type
- What matters most to you, such as premium, deductible, doctors, prescriptions, “Maternity or planned surgery,” or dental and vision
- Doctors or hospitals you want to keep, and regular prescriptions with dosages
- Monthly budget, preferred start date, and recent life changes, such as a move, marriage, new child, job loss, or income change
- Your name, email, mobile number, and best time to reach you
- Whether you checked our consent box for calls and texts
Nothing on the quote form reaches us until you press “Get my quote” on the Contact step. Some questions start with a default answer, such as Florida for state. If you leave a default unchanged, we receive that answer.
On our contact form, you tell us

- Dates of birth for each person who needs coverage
- Who needs coverage: you, your family, a self-employed person, or a business
- Anything you write in the message box, such as doctors, prescriptions, or budget

Your consent record. When you submit a form, we save:

- The date and time, taken from your device's clock

- Your IP address and browser details, which our website host records
Collected automatically
- Our website host, Netlify, records your IP address, browser details (user agent), referring page, and time with each form submission.
- Netlify also receives your IP address and browser details whenever you load a page.
- Google Fonts receives your IP address and browser details when our pages load their fonts.
When you call, text, email, or meet with us
- Anything you tell us so we can compare plans, such as household income, current coverage, and health needs
- Text messages and emails you send us, and our replies
- Booking details you enter on Calendly when you schedule a call
- We do not record calls or online meetings.
When you apply for coverage
- Information the application needs, such as income, current coverage, and Social Security number
- We never ask for your Social Security number or bank details to give you a quote. When you apply, they go straight into the enrollment application: the insurance company's, the Marketplace's, or HealthSherpa's, an approved Marketplace enrollment platform. We do not keep copies.
From other people and sources
- Information you give us about your spouse, children, or other household members
- For group coverage, an employee census from the employer. It lists each employee's name, date of birth, home ZIP code, gender, and whether they would cover a spouse or children.
- We do not need employee health information to quote group coverage. For level-funded plans, medical underwriting happens later, with the insurance company, not with us.
- From insurance companies and the Marketplace: plans you applied for, eligibility results, enrollment status, policy numbers, and premiums
We do not order credit reports or prescription-history reports. We also do not order Medical Information Bureau (MIB) reports, which insurers use to check health history.
If you give us information about another person, make sure you are allowed to share it. A parent or guardian should provide a child's information.
Health information
Some information you give us is health information. This includes:
- Doctors and hospitals you use
- Prescriptions and dosages
- Tobacco use
- Planned care, such as pregnancy or surgery
- Any health details you write in a message or tell us on a call
On the quote form, the “Doctors or hospitals you want to keep,” “Regular prescriptions,” and “Maternity or planned surgery” answers are optional. Skipping them may make our plan comparison less precise.
We use health information only to recommend, quote, enroll, and service coverage for you. That means we:
- Check your doctors and prescriptions against plan networks and drug lists
- Compare, quote, and enroll you in coverage you ask about
- Service your coverage after you enroll, such as re-checking your doctors and drugs at renewal
If you do not enroll, we do not send you follow-up marketing, so your health details are never used for it.
We share health information only:
- With the insurance companies or Marketplace you ask us to quote or apply with
- With service providers that store or process it for us, such as our website host and email provider
- With our lawyers, accountants, auditors, and professional liability insurer, who must keep it confidential
- When a law, court, or insurance regulator requires it, or when needed to establish, exercise, or defend legal claims
- If our agency is sold or merged, as described in “How we share your information”
We never sell health information. We never share it with anyone for their own marketing. We never give it to other agents, lead buyers, or advertisers.
If we ever wanted to share it for any other reason, we would first ask for your written authorization. That authorization would name who gets the information and why. It would last no more than 24 months. You could revoke it at any time. Saying no would not change how we treat you.
State insurance privacy rules and the promises in this policy protect your health information. HIPAA, the federal health privacy law, mainly covers doctors, hospitals, and health plans. So it usually does not cover what you type on an agent's website. We may sometimes work for an employer's health plan or an insurance company under a HIPAA contract, called a business associate agreement. When that happens, HIPAA's rules also apply to that information.

- Keep records of your consent and your opt-out requests
- Filter spam and keep our systems secure
- Meet our legal, licensing, and recordkeeping duties, and defend legal claims
We do not build advertising profiles about you. We do not use your information for targeted advertising.
How we share your information
We share your information only with these groups, and only as needed:
- Insurance companies you ask us to quote or apply with. They use it to quote, enroll, and service your coverage. For some products, they also use it to decide whether to accept your application.
- The Health Insurance Marketplace (HealthCare.gov) or your state's exchange, when you apply for a Marketplace plan through us.
- Service providers that run parts of our business for us:
- Netlify hosts our website and stores form submissions. It checks every submission for spam with Akismet, a service of Automattic. It uses other companies to deliver email.
- Google provides our email (Gmail). Each form submission is emailed to our inbox. Google also delivers our website fonts.
- Calendly runs our appointment booking.
- We call and text from our own mobile phone, (603) 401-9695. Our phone carrier delivers those calls and texts, and the phone keeps our text history and contacts.
- A video meeting service lets us share our screen during consultations.
- HealthSherpa, CloseQuote, and insurance companies' own broker websites help us quote, check networks, and enroll coverage.
- Our calendar and our phone's cloud backup store appointments, contacts, and texts.
- Our professional advisers: our lawyers, accountants, auditors, and professional liability insurer, who must keep it confidential.
- Government agencies, insurance regulators, and courts, when the law requires it. We may also share it to establish, exercise, or defend legal claims. That includes producing consent and call records.
- A buyer or successor. If our agency is sold or merged, your information may go with the buyer or successor. We will tell you first.
We use established service providers whose published terms limit how they use the information we store with them. Akismet says it keeps most spam-related data for no more than 90 days. It also uses content flagged as spam to improve its filter for other websites.
Calendly runs our scheduling for us. When you book, we receive the details you enter. Calendly also sets its own cookies on its site.
We do not share your information with anyone else. We have no affiliates (companies owned by or connected to us) that receive it. Because we share only for the reasons above, you have nothing to opt out of. These rules also apply after you stop being our client.
Calls and texts
We use your mobile number to call or text you about your quote, appointments, and coverage. If you become our client and agreed, we may also text you about renewals, open enrollment, and other coverage that may fit. We never send marketing texts or emails to people who did not buy from us.
- Consent to calls and texts is not a condition of buying anything. You can call us at (603) 401-9695 instead.
- We never sell, rent, or share your mobile number, text opt-in data, or consent with third parties or affiliates for their marketing or promotional purposes. This includes lead generators.
- Your text opt-in and consent records are handled only by the service providers that host and filter our forms and deliver our email. They handle them only to provide those services. We text from our own mobile phone, not from a texting platform.
- We may also give consent records to our lawyers, a court, or a regulator when the law requires it or to defend a legal claim.
- Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to cancel.
- You can take back your consent at any time, by any reasonable means. Reply STOP to any text, or tell us by phone, text, or email.
- After you text STOP, we will not text you again.
- Opt-outs made any other way take effect promptly, and no later than 10 business days.
- One opt-out from marketing stops all of our marketing calls and texts.
- We keep a do-not-call list. Your request stays on it for at least 5 years. Our do-not-call policy is part of our Terms of Use.
Please do not text us Social Security numbers, bank or card numbers, or detailed medical information. Text messages are not a secure way to send them.

- Our own website code sets no cookies.
- We do not use analytics, advertising pixels, or tag managers.
- Our pages load no tracking tools from other companies. The only outside service they load is Google Fonts.
- Netlify and Google Fonts receive your IP address and browser details, as described in “Information we collect.” They need them to deliver our pages and fonts.
- Calendly's booking page is a separate site. Calendly sets its own cookies there. Its privacy notice says it may let other companies collect information through cookies on its site.
We do not track you across websites, so Do Not Track signals change nothing on our site. We treat a Global Privacy Control signal as a valid request to opt out of sale and of sharing for targeted advertising. We never sell your information or share it for targeted advertising, so honoring the signal changes nothing. See our Do not sell or share my information page.
Links to other websites
Our site links to HealthCare.gov, Calendly, and other websites. Their own privacy policies apply on those sites. The exception is the booking details Calendly collects for us. This policy covers those.
How long we keep information
- Consent records (the wording shown, time stamp, page address, IP address, and browser details): at least 5 years.
- Do-not-call and opt-out requests: at least 5 years from the date of the request.
- Marketplace consent records: at least 10 years, if we help you with a Marketplace application.
- Client files: while you are our client, and at least 5 years after.
- Inquiries from people who do not become clients: up to 5 years, because each one includes your consent record.
- Text messages and emails: as long as we need them to serve you and meet our record-keeping duties.
When we no longer need information, we delete or destroy it so it cannot be read. That includes copies with our website host, in our email, calendar, and phone, and in exported files.
How we protect your information
- Our website uses HTTPS. It encrypts form data between your browser and our website host.
- Only our licensed advisor can access form submissions and client files.
- We use two-step sign-in on our email and website-hosting accounts.
- We do not keep copies of Social Security numbers, income documents, or payment details.
No system is perfectly secure. If a breach affects your information, we will notify you within the time the law requires.
Your rights and choices
You can ask us to:
- Tell you what personal information we have about you, and give you a copy
- Tell you who we shared it with in the past 2 years. If we did not record that, we will tell you the kinds of companies we normally share with.
- Correct information that is wrong or incomplete
- Delete your information
- Limit your health information. We would then collect and use it only to finish what you asked for, service coverage you have, and keep records the law requires.
- Stop calls, texts, or emails
- Revoke any authorization you gave us
If you ask us to stop using your health information entirely, we may not be able to check your doctors and prescriptions, quote, or service your coverage.
How to ask. Email CammaranoHealthSolutions@gmail.com, call or text (603) 401-9695, or write to us at 5701 E Hillsborough Ave, Ste 1202, Tampa, FL 33610. Some states ask for access and correction requests in writing. Email counts as writing.
Confirming it's you. We match your name, phone number, and email to our records. We may ask a question only you would know. You may use an authorized agent, meaning someone you choose to act for you. We may ask for proof of the agent's authority.
Response time. Requests to stop calls, texts, or emails take effect within 10 business days. Texting STOP stops our texts as soon as we read it. We answer other requests, such as a copy, a correction, or a deletion, within 30 business days.
Corrections. If you ask, we will send the correction to anyone our records show received the old information from us in the past 2 years. If we decline a correction, we will tell you why in writing. You may then send a short statement explaining why you disagree. We will keep it with your file and include it whenever we share that information.
Deletion limits. We keep records we must keep by law. These include consent records, opt-out records, client files, and Marketplace consent records. We cannot delete information an insurance company or the Marketplace holds. Contact them directly.
If you disagree with our answer, reply and ask us to reconsider. You can also contact your state's department of insurance or attorney general.
We will not treat you differently for using any of these rights.
Marketplace privacy notice
This section applies when we help you apply for a Marketplace plan or for help paying for coverage.
- Authority. We are a licensed agent registered with the Health Insurance Marketplace. Federal rules allow us to collect your information to help you apply and enroll.
- Purpose. We use it to check your eligibility for Marketplace coverage and savings, and to enroll you in a plan.
- Who receives it. Information on your Marketplace application goes to the Centers for Medicare & Medicaid Services (CMS), a federal agency. CMS keeps it in a System of Records, a federal records system protected by the Privacy Act. The insurance company you choose also receives it.
- Limited use. We use Marketplace information only for these purposes. Any other use needs your explicit consent.
- Voluntary. Giving us information is voluntary. Without it, we may not be able to help you apply or enroll.
- Your consent. Before we help you, we get your consent and record it. We keep that record for at least 10 years.
- Security. We protect it with the safeguards in “How we protect your information” on this page.
- Complaints. You can complain to Cammarano Health Solutions by email at CammaranoHealthSolutions@gmail.com, or by phone or text at (603) 401-9695. You can also contact the Marketplace Call Center at 1-800-318-2596 (TTY 1-855-889-4325).
Children
Our website is meant for adults 18 and older. We do not knowingly collect information directly from children. A parent or guardian may give us a child's name and date of birth to quote family coverage.
If you live outside our licensed states
We are licensed only in the 26 states on our Licenses page. If you choose “My state isn't listed,” we cannot sell you a policy. We will use your information only to reply to you. We will not pass it to another agent.
Copies for clients
If you become our client, we send you a copy of this policy when you enroll. We send it again at least once a year while you remain a client. We send it by email if you agree to email delivery, or by mail.
Changes to this policy
We may update this policy. We will post the new version here with a new effective date. If we make a material change, we will also post a notice on our home page. We will email current clients before we change how we share information. We will not start sharing in a new way without notice and any consent the law requires.
This never applies to your mobile number, text opt-in data, or consent. We will never sell or share those with third parties or affiliates for their marketing or promotional purposes, even with permission.

These Terms of Use apply when you use cammaranohealth.com. They also apply to our text-message program. “We,” “us,” and “our” mean Cammarano Health Solutions. “You” means the person using our site or services.
By using our site, you agree to these terms. If you do not agree, please do not use the site.
Our Privacy Policy explains how we handle your information.
Who we are
- We are Cammarano Health Solutions LLC, an independent, licensed health insurance agency based in Tampa, Florida.
- Our licensed advisor is Gerardo Cammarano. His National Producer Number (NPN) is 21778286. His Florida resident license number is G291752.
- We are licensed in 26 states, listed on our Licenses page. We can sell insurance only in states where we hold a license.
- We are not a government agency. We are not affiliated with or endorsed by the U.S. government, CMS, HHS, or any state agency.
- This site is not the Health Insurance Marketplace website. To see all of your options, visit HealthCare.gov or your state's Marketplace.
- We do not offer every plan available in your area. We can offer plans only from insurance companies that have appointed us.
Information on this site
- The site gives general information. It is not a quote, an offer of coverage, or a promise of eligibility.
- Plan availability, benefits, networks, and premiums vary by state, county, plan year, and applicant. They can change.
- Coverage starts only after the insurance company or Marketplace accepts your application or enrollment, and you pay any required first premium. Your policy documents control in every case.
- Subsidy and cost estimates depend on the information you give us. The Marketplace or insurance company makes the final decision.
- We do not give tax or legal advice. Please ask a tax professional about tax questions.
- Short-term, fixed indemnity, accident, critical illness, and hospital indemnity products are not major medical insurance.
- Those products do not have to meet certain Affordable Care Act requirements. They may exclude pre-existing conditions. They may not satisfy coverage requirements. They are not sold in every state.
How we are paid
Our consultations are free. Insurance companies usually pay us a commission when you enroll. For individual and fully insured group plans, premiums are filed with each state. You pay the same premium for those plans with or without us. You may ask us how we are paid for any plan we recommend. We never charge you a fee.
Using our forms
- You must be at least 18 years old.
- Give true and complete answers. Wrong answers can lead to wrong quotes or problems with an application.
- Give information about other people only if you are allowed to. A parent or guardian should give a child's information.
- Enter only a phone number you own or are allowed to use.
- Submitting a form does not enroll you in any plan.

If you text us first: We will reply to that conversation only. That does not sign you up for our text program or for marketing texts. To get them, check the consent box on our forms.

How often: Message frequency varies.
Cost: Message and data rates may apply. Ask your wireless carrier about your plan.
Who writes the texts: We send no automated or bulk texts. Gerardo writes and reads every text himself.
How to stop
- Reply STOP at any time. QUIT, END, REVOKE, OPT OUT, CANCEL, and UNSUBSCRIBE also work.
- Texting STOP ends all texts from us, including appointment reminders. We stop as soon as we read it.
- After that, you will get no more texts unless you join again.
- To stop only marketing texts and keep appointment and coverage texts, tell us.
- You can also opt out any other reasonable way, such as by phone, email, or mail. We act on those requests promptly, and no later than 10 business days.
- One opt-out from marketing stops all of our marketing calls and texts.

Not a condition of purchase: You do not have to agree to texts to buy any product or service.
Delivery: Wireless carriers are not liable for delayed or undelivered messages.
Who can join: You must be 18 or older. You must be the account holder or an authorized user of the phone number.
Changing your number: If you change or give up your number, text STOP first or tell us. That way the new owner will not get our texts.
Keep sensitive data out of texts: Do not text Social Security numbers, bank or card numbers, or detailed medical information. Text messages are not a secure way to send them.
Privacy: We never sell, rent, or share your mobile number, text opt-in data, or consent with third parties or affiliates for their marketing or promotional purposes. Our Privacy Policy explains how we handle your information.
Phone calls and meetings
- We call between 8 a.m. and 8 p.m. in your local time, or within any narrower hours your state sets.
- We do not call more than three times in 24 hours about the same matter.
- On every sales call, our advisor gives his first and last name, our business name, and a number where you can reach us.
- Our caller ID shows a number you can call back.
- We keep an internal do-not-call list. If you ask us not to call or text you, we add you to it promptly, and no later than 10 business days, and keep you on it for at least 5 years. This section is our written do-not-call policy.
- During a consultation, we may share our screen so you can see the plans we compare.
- We do not record calls or meetings.
Booking with Calendly
We use Calendly to book appointments. Calendly runs our scheduling for us. Calendly's own terms apply on its site, and it sets its own cookies there.
Acceptable use
When you use our site, please do not:
- Submit false information, or someone else's information without permission
- Enter a phone number or email address that is not yours to use
- Send spam or automated submissions through our forms
- Try to break into, overload, or disrupt the site
- Copy or scrape the site with automated tools

Who does receive your information
We share information to help with the coverage you ask about and to run our business. That means:
- The insurance companies and Marketplace you choose to quote or apply with
- Service providers that run our website, email, scheduling, phone, video meetings, and quoting and enrollment tools for us
- Our lawyers, accountants, auditors, and professional liability insurer, who must keep it confidential
- Regulators or courts, when the law requires it, or to defend a legal claim
- A buyer or successor, if our agency is sold or merged. We will tell you first.
None of these is a sale, and none is for targeted advertising. Our Privacy Policy lists each one.
Browser privacy signals
Some browsers and extensions send a Global Privacy Control (GPC) signal. It tells a website not to sell or share your information.
